8 am – 5 pm schedule with minimal call shared among all state providers
RN call center handles calls, only urgent matters reach physicians
Value-based care experience with HEDIS measures and HCC coding preferred
Work with complex geriatric patients aged 65+ in integrated care model
Collaborative team with 2 very strong advanced practice providers
eClinicalWorks experience preferred for top pay scale consideration
Board-certified or board-eligible in geriatric medicine required
Spanish fluency a plus but not required for this urgent position
